If you would like to apply to be placed on the Canadian Consensus Health Benefits Scholarship Program, you will need to fill out our Form 3D application form which can be found here. All participants will need to make an initial deposit of $6,500 before making your application online. Please note that if youre new to Canada, you may choose not to live here. The provincial government will cover all expenses involved for people who do not live here, but you must also fill out our form to be admitted to live here if you havent worked in the provinces.
